Info: Healthcare in Action (HIA) is a medical group that provides care to older adults and other vulnerable individuals who are experiencing homelessness. HIA serves patients experiencing homelessness through an innovative “street medicine” approach. They achieve this by contracting with Medicare and Medicaid health plans, as well as hospitals and health systems. Instead of waiting for patients to come to a clinic, HIA’s healthcare providers actively go out into the community to deliver care directly to those in need. This approach ensures that homeless individuals receive essential medical services, even if they don’t have a fixed address or access to traditional healthcare facilities. The dedicated team at HIA includes physicians, nurses, and other healthcare professionals who work together to provide holistic and empathetic care. They address not only physical health but also mental health and social well-being. HIA’s commitment to serving homeless populations exemplifies their dedication to making healthcare accessible and compassionate for everyone, regardless of their circumstances.
