CoC Grants

The Continuum of Care (CoC) is a regional or local planning body that coordinates housing and service funding.

State CoC Homeless Housing, Assistance and Prevention Program (HHAP)

HHAP is a one-time State block grant program designed to provide jurisdictions with one-time grant funds to support regional coordination and to expand or develop local capacity to address immediate homelessness challenges throughout California.  OHS is the designated AE to administer HHAP on behalf of the SBC CoC.

Continuum of Care Builds

The Continuum of Care Builds (CoC Builds) Program is a competitive grant program administered by the U.S. Department of Housing and Urban Development (HUD). CoC Builds targets efforts within a CoC geographic area to address and reduce persons experiencing homelessness by adding new units of Permanent Supportive Housing (PSH) through new construction, acquisition, or rehabilitation through one-time CoC Builds awards under the CoC Program.

Housing and Homelessness Incentive Program – CoC

HHIP is a statewide voluntary program in which Managed Care Plans (MCPs) work at the county level with local community stakeholders in building partnership with social service providers and homeless systems of care. Locally, the Inland Empire Health Plan (IEHP), in consultation with the County and SB CoC, developed an Investment Plan that allocates $10 million towards five investment areas, in which the ultimate intent is to improve health outcomes and access to whole-person care services by addressing housing insecurity and instability as social determinants of health for the Medi-Cal population (Members).

CoC Continuum of Care 2022 Supplemental Funding Opportunity to Address Unsheltered and Rural Homelessness

The U.S. Department of Housing and Urban Development (HUD) announced the release of the Continuum of Care (CoC) Supplemental to Address Unsheltered and Rural Homelessness (Special NOFO) on June 22, 2022. Approximately $322 million is available nationally ($267.5 million for the Unsheltered Homelessness Set Aside and $54.5 million available for the Rural Set Aside) to create new projects to reduce unsheltered homelessness.

HUD CoC Competition

Each year the U.S. Department of Housing and Urban Development (HUD) announces a Notice of Funding Availability for the CoC Program Competition.  The CoC Homeless Assistance Program is designed to promote a community-wide commitment to the goal of ending homelessness and to provide funding for efforts by nonprofit providers, states, and local governments.  The Office of Homeless Services (OHS) is the administrative agent for the San Bernardino County Homeless Partnership. OHS acts as the agent to submit the CoC Consolidated Application to HUD for the entire San Bernardino City and County CoC.

State CoC Homeless Emergency Aid Program (HEAP)

HEAP is a one-time State block grant program to provide direct assistance to cities and counties to address the homelessness crisis throughout California.  The San Bernardino County CoC (SBC CoC) is the eligible applicant for the State HEAP funding and OHS is the designated Administrative Entity (AE) to administer HEAP on behalf of the SBC CoC.
